2. Purchase of Form 10 at the Ministry of Interior ( Accra ) or at the Consular Section in any of the Ghana's Diplomatic Mission Abroad

4. Evidence of current Nationality



5. Evidence of Parent's Ghanaian Nationality ( Passport)If applicant is a minor ( Below 18 years)
   
   
6. Full residential address and contact numbers of Two (2) Ghanaian relatives living in Ghana



7. Six (6) Passport Photographs
